I hereby authorize the directors of the camp to use their best judgment in any emergency for medical attention. I acknowledge that the Rockport-Fulton Summer Camp does not provide medical insurance covering injuries of any nature incurred during the camp. I also acknowledge that I will be responsible for any cost (through family medical insurance or otherwise) incurred due to sickness or injury to my child. I hereby waive any claim I might have against Rockport-Fulton Independent School District and/or the Rockport-Fulton Summer Camp Directors.
